University Policy 1204 Report Form

If you have information about a possible violation of University Policy 1204, you are encouraged to report it promptly. The University is committed to maintaining an environment that upholds the highest ethical and professional standards, and your report helps ensure compliance with this policy.


What Should Be Reported?

You should report any suspected violations of Policy 1204, including:

  • Prohibited Relationships – Any relationship that falls under the University's definition of a Prohibited Relationship, including those between faculty, staff, student employees, or volunteers and students over whom they have—or may reasonably be expected to have—responsibility.

  • Attempts to Initiate a Prohibited Relationship – Any effort by an employee, student employee, or volunteer to begin a Prohibited Relationship with a student.

  • Failure to Report a Prohibited Relationship – Employees, student employees, and volunteers are required to disclose relationships as outlined in Policy 1204. If you are aware of a relationship that has not been reported, this should be brought to the University’s attention.


How to Report a Violation

If you suspect a violation, you can report it by submitting the Policy 1204 Violation Report Form. Reports can also be made directly to the appropriate University office responsible for reviewing compliance with this policy.

All reports will be reviewed by the Report Review Committee, which will determine whether a violation has occurred and what actions may be necessary. Reports may also be referred to Human Resources, the Academic Compliance Committee, or other relevant University offices for further investigation.


Confidentiality and Protection from Retaliation

The University takes all reports seriously and will handle them as confidentially as possible. Retaliation against individuals who report suspected violations in good faith is strictly prohibited and may result in disciplinary action.


Consequences of Policy Violations

Violations of Policy 1204 may result in disciplinary action, up to and including termination. This includes engaging in a Prohibited Relationship, attempting to initiate one, or failing to report an existing relationship as required.
